current or expired Pennsylvania Training License? If
yes, Please go to 'B' or 'C'.
If no, please follow instructions 1-5 below.
1. Choose the
applicable application form below.
2. Read the
instructions provided with the application.
3. Please use
your CURRENT home address on this form.
4. Mail
completed Pages 1 and 2 with a check made payable to the
“Commonwealth of PA” to
your program. (The program director will then sign
the bottom of Page 1, and submit the
application to the Pennsylvania State Board of
Medicine.)
5. Mail Page
3 to your medical school for completion. (The
medical school must submit
verification directly to the State Board of
Medicine.)
NO RESIDENT WILL BE
PERMITTED TO ROTATE AT PENN STATE HERSHEY UNLESS HE/SHE HAS SECURED A TRAINING
LICENSE
IN PENNSYLVANIA.
