Itemized Cost of Attendance Evaluation Form
Complete this form to request an increase to your cost attendance for Off Campus Housing, Childcare or Computer Purchase. Mail or fax the completed form to the Office of Student Aid.

Free Application for Federal Student Aid (FAFSA)*
You need to complete this form to begin the process of obtaining financial assistance. This form is required for loans, grants and scholarships. You must complete the parental information section on this form to be considered for institutional loans and scholarships.

Special Circumstance Evaluation Form **

If you have a change in circumstances (other than becoming married) that will cause a significant change in your financial situation (such as leaving a job to enter medical school), you may want to complete a Special Circumstance Evaluation Form. This form allows the Student Aid Office to take these changes into consideration when determining your financial aid eligibility. Special Circumstance Forms are evaluated in July of the year that you matriculate.

  • Print this form.
  • Complete the appropriate information and attach the required documentation.
  • Return the form and required documentation to the Student Aid Office.
