Health/Insurance

STUDENT HEALTH

Health care is available to students and their immediate families through Student Health Services, based in the Department of Family and Community Medicine.  The comprehensive primary care coverage includes acute and chronic care for any and all medical problems, preventive healthcare services including GYN and Family Planning services, patient advocacy role for any referrals to specialty services, a confidential referral system for counseling needs in association with the Department of Psychiatry, and a 24-hour call coverage system.  To make an appointment please call (717) 531-5998 .   The office of Student Health is based at the University Physician Group-Fishburn Road, 845 Fishburn Rd., Hershey, PA.  The secretary/scheduler, Janice Mesarick, is available at (717) 531-5998 from 8-5 PM Monday through Friday.  After 5 PM Monday through Friday, as well as Saturday and Sunday, healthcare concerns are forwarded to the hospital nurse triage system. If the nurse triage system has concerns they are forwarded to the Family Medicine Physician on call.  If you need assistance in traveling to Student Health (you do not have a car or are too ill to drive yourself), please call the shuttle driver at 717-514-4546 to arrange transportation. 

Health Insurance

ATTENTION:  GRADUATE ASSISTANTS AND FELLOWS FOR FALL 2007: 

  • You (student only) will be automatically enrolled in the Penn State GA/Fellow Health Insurance Plan, but can elect to decline this coverage online.  Deadline is no later than September 10, 2007
    Insurance is mandatory for international students and their accompanying dependents.

GA/Fellow Online Declination Forms

  • Eligible spouse and/or eligible children MUST be manually enrolled (not automatic).  Dependents must be enrolled each fall semester.  Deadline is no later than September 10, 2007

MEGA

Graduate Assistants and Graduate Fellows* it is intended that this site will provide you with the necessary information and links to get you acquainted with your health insurance benefit, the Penn State Graduate Assistant and Graduate Fellow Health Insurance Plan.

Amount of Health Insurance Premium Subsidy

As a Graduate Assistant or Graduate Fellow, you are eligible to receive subsidies in the amount of 80% of the annual premium cost for the Graduate Assistant and Graduate Fellow Insurance Plans. The university will pay this amount directly to the insurance companies and will deduct your 20% contribution to premium expense from your monthly check.

Your insurance subsidies for your eligible dependents is 70% of the annual premium expense. As with the subsidy for your individual insurances, the university will pay 70% of the premium expense directly to the insurance companies and you will pay your 30% of the premium costs through a payroll deduction. Dental Insurance

As a Graduate Assistant or Graduate Fellow, you are eligible to receive a subsidy in the amount of 80% of the annual premium cost for the United Concordia Dental Insurance Plan. Your insurance subsidy for your eligible dependents is 70% of the annual premium expense.


Vision Insurance

As a Graduate Assistant or Graduate Fellow, you are eligible to receive a subsidy in the amount of 80% of the annual premium cost for the National Vision Insurance Plan. Your insurance subsidy for your eligible dependents is 70% of the annual premium expense.

Ophthalmology Clinic at Hershey Medical Center (student only)- x5690
**Even without vision insurance, graduate students can receive significant discounts at the Ophthalmology Clinic (at the University Physicians Clinic).
