Children's Miracle Network Sales Room H1260 - Reservation Request Form

To reserve the Sales Room for your event, please complete the reservation request form and return to the Children's Miracle Network office via fax (717-531-0888) or e-mail.

*Before submitting the form, please review the policy/procedure for Children's Miracle Network Sales Room within the Hospital Administration policies.  If you are a Non-University Group or a University Group who is bringing a vendor or outside party on to campus, review the Guidelines for Use of Penn State Milton S. Hershey Medical Center Campus Facilities by Non-University Groups.

*Please note:  Departments, employees, students and vendors are not permitted to attach (tape, pushpin, or any other means) notices, advertisements, decorations, etc. on walls, doors or any other building surfaces.  These items may only be placed on hospital bulletin boards.  All violators will be charged for repairs.  In addition, you are responsible to remove all notices, advertisements, decorations, etc. from campus at the conclusion of your event.
