Posttest and Program Evaluation


Evaluation of this activity is integral to the CME process. CME certificate requests cannot be processed without the posttest and evaluation form.  Participants must receive a score of 80% or better to receive credit.  Please submit your materials within two weeks of the teleconference. Materials must be received by January 31, 2009. After this date the activity will no longer be designated for credit.  A CME certificate will be mailed within 6 to 8 weeks.  It is recommended that you keep a copy of your completed materials until you receive your certificate.
